Does pet insurance cover pyometra in dogs?

By Bradley Jacobs · Published Oct 25, 2026 · Last checked Oct 26, 2026

Often yes, if the infection starts after your policy and its illness wait. But this one has more fine print than most. Some policies leave out anything tied to breeding, and some care about whether and when your dog was spayed.

What pyometra is

Pyometra is a bad infection in the womb. It happens almost only in female dogs who are not spayed.

Lemonade says it usually shows up 2 to 8 weeks after a heat. It is an emergency. The usual fix is surgery to spay her and take out the infected womb.

Signs to call your vet about the same day:

  • Tired and not herself.
  • Drinking and peeing a lot more.
  • Not eating, or throwing up.
  • A swollen belly.
  • Smelly discharge from her private area (not every dog has this).

The short answer

  • It is an illness, not an accident. So the illness wait applies. Lemonade and Embrace say 14 days. Fetch says up to 15.
  • Lemonade says it may cover it. That means tests, surgery, and the hospital stay, if signs first show after the wait and it is not already in her records.
  • Breeding exclusions are the big question. Embrace lists breeding, whelping (giving birth), and pregnancy as not covered. Ask how a carrier treats pyometra in a dog who was never bred.
  • Your vet's advice matters. Lemonade says turning down a spay your vet clearly and repeatedly recommended can affect a claim.
  • Accident-only plans will not help here. They pay for injuries, not infections.

The spay and breeding fine print

This is where pyometra is different from most illnesses. Read these parts of any policy before you buy.

Breeding exclusions

  • Most policies leave out costs from breeding, pregnancy, and giving birth.
  • Pyometra is not pregnancy. But it only happens because the womb is still there, so the wording matters.
  • Ask the carrier to tell you in writing which way they read it.

Spay-by-age rules

  • An older Trupanion sample policy we found says: if a pet was not spayed before her first birthday, womb and ovary problems are not covered.
  • That same policy makes an exception if the spay timing followed your vet's advice, or if she was spayed within 30 days of adoption.
  • That form is dated 2010. Today's Trupanion policy may read differently. Verify on the quote form and the issued policy.

The spay itself

  • A planned spay on a healthy dog is routine care. The main plan does not pay for it.
  • Some wellness add-ons help pay toward it (Lemonade, Fetch, and Spot each list one). See accident-only vs wellness.
  • An emergency spay to treat the infection is treatment for an illness. Ask the vet to write the reason on the bill.

Older unspayed dogs

Lemonade says middle-aged to older unspayed females are most at risk. The risk goes up with each heat.

  • If she had discharge or a womb problem noted before signup, a carrier will likely call it pre-existing.
  • Many carriers read about the last 12 months of records. Trupanion looks back 18 months.
  • Lemonade says it is not too late to ask your vet about spaying an older dog. Spayed dogs can still get a rare leftover-tissue infection, but the risk drops a lot.

Age caps matter too. See enrollment age limits, senior dogs over 8, and problems already in the record.

What it can cost

We do not make up prices. Lemonade says emergency pyometra care often runs about $1,500 to $5,000 or more.

That covers tests, surgery, the hospital stay, fluids, and medicine. It can climb higher if the infection spreads. Your premium is on the quote form, not here.

Carrier-by-carrier (public pages)

These notes come from carrier public pages we read in October 2026. They are not personal underwriting.

Anything can go stale. Verify on the quote form and the issued policy.

  • Lemonade:
    • Has a pyometra page that says a policy may cover tests, surgery, and hospital stay.
    • Signs must first show after the 14-day illness wait.
    • Turning down a spay your vet repeatedly recommended can affect a claim.
  • Embrace:
    • Its FAQ lists breeding, whelping, and pregnancy as not covered.
    • We did not find a pyometra page. Ask how they treat it in a dog never bred.
    • 14-day illness wait.
  • Trupanion:
    • An older sample policy (dated 2010) leaves out womb and ovary problems if she was not spayed by her first birthday, with exceptions for vet advice and new adoptions.
    • Looks back 18 months for pre-existing signs. Verify today's wording.
  • Fetch:
    • Illness wait of up to 15 days.
    • Spay is paid only through its wellness add-on.
    • We did not find a pyometra coverage page. Ask on the quote form.
  • Pets Best:
    • No upper age limit to enroll.
    • We did not find a pyometra page. Ask how breeding exclusions apply.
  • Spot:
    • Spot's own sample policy leaves out costs from breeding, pregnancy, whelping, or nursing.
    • It does not name pyometra. Ask how they read it.

Questions to ask before you buy

  • My dog is not spayed and was never bred. Is pyometra covered?
  • Does your breeding or pregnancy exclusion include womb infections?
  • Do you have a rule about spaying by a certain age?
  • If I spay her later, does that change anything?
  • How long is the illness wait in my state?
